What is HITRUST and who needs it?

HITRUST is a control framework with e1, i1 and r2 validated assessments. Who it fits, how the levels differ and how to tell whether a customer will ask.

HITRUST is a security and privacy control framework, the HITRUST CSF, with validated assessments built on it: e1, i1 and r2. Organizations need it mainly because customers, often in healthcare, ask for it. Check what your buyers require before you spend on it.

What is the HITRUST CSF?

HITRUST describes the CSF as a unified control library that harmonizes more than 70 standards and regulations into one framework. It maps controls to sources such as ISO/IEC 27001 and 27002, NIST SP 800-53 Rev. 5, HIPAA, PCI and GDPR. Every HITRUST assessment is built on it.

HITRUST also lists an AI Security Assessment and an AI Risk Management Assessment, the latter with 51 controls harmonized with ISO 23894 and the NIST AI RMF. Those are separate products from the three core cybersecurity assessments.

How do the three assessments differ?

HITRUST publishes a traversable portfolio, so work done at one level can be reused at the next.

e1 is a one year validated assessment based on 43 foundational security controls. HITRUST calls it ideal for startups and companies with limited risk profiles or less complexity.

i1 is also a one year validated assessment. It suits organizations with mature security programs in place that want to show leading practices, and it includes more controls than the e1. Work done for an active i1 can count toward an r2.

r2 is a two year validated assessment for organizations that need to demonstrate compliance with authoritative sources such as HIPAA and the NIST Cybersecurity Framework, or that need expanded tailoring of controls based on risk factors. HITRUST calls it the most comprehensive of the three.

HITRUST's materials say a certified organization can inherit controls from certified cloud providers, which can reduce the work.

Who actually needs it?

Nobody needs it by law. The pressure is commercial. HITRUST's own site says many organizations request or require certification from vendors and service providers. If you sell to health systems, insurers or other buyers who write it into vendor requirements, you will be asked. If no customer has asked, an alternative framework may serve you better for less.

Ask three questions. Which of your target customers name HITRUST in security reviews? Will they accept a different attestation, such as SOC 2 or ISO 27001? Do you have enough operating history to show controls working?

What should you be skeptical of?

HITRUST's pages cite breach statistics for certified environments. Those figures are the organization's own claims about its program, so read them as marketing and do not reuse them as proof for your customers.

Also keep in mind that a validated assessment is performed with assessors and takes real effort across engineering, IT and people operations. Plan for the work, not just the fee.

What to do

  1. Collect the questionnaires and contract clauses your buyers send, and note which name HITRUST.
  2. Ask those buyers which level they expect, e1, i1 or r2.
  3. Compare against other frameworks you may already be working toward.
  4. If you proceed, scope the assessment narrowly to the systems that serve those customers.
  5. Run a readiness review against the controls before booking the assessment.
  6. Reuse the work: start at e1 only if the buyer accepts it, since lower levels can feed higher ones.
  7. Put renewal dates on the calendar, one year for e1 and i1, two for r2.

Quick check

Is HITRUST the same as HIPAA compliance?

No. HIPAA is a law. HITRUST maps to it and to other sources, but certification does not mean a regulator has found you compliant.

Can a startup start with e1?

HITRUST positions e1 for startups and lower risk profiles. Confirm that your customers will accept it.
